Course Enrollment

Thank you so much for filling out the diagnistic information so far. Happy Academy offers a variety of courses for all grades. Please fill out the following and list what courses you are interested in enrolling for.

Transcripts, Awards, and Photo Release Form

The following is required to complete enrollment. Please review, check and sign the following content.

Most applications to colleges, universities, high schools, summer programs, ROTC programs, service academies, NCCA Eligibility Center, scholarship programs, and potential employers, require that an official transcript of your academic history at Happy Academy be forwarded by the school on your behalf. In order to process any such application or registration, the following statement must be signed.

I authorize the release of a copy of my official transcript containing a list of courses I have taken, or attempted, and grades earned as well as any other educational records, as required or requested by the institutions to which I apply. 

I understand that standardized test scores do not appear on my transcript and are not forwarded by Happy Academy, and that I must contact the respective testing service(s) to have my scores sent to any institution. 

Happy Academy considers the electronic signature(s) below to be equivelant to a handwritten signature and agreement to the terms above.

I authorize the use of photos taken of me during my time at Happy Academy as identified below for presentation under any legal condition, including but not limited to: publicity, copyright purposes, illustration, advertizing, and web content: I understand that there shall be no payment nor royalties for this release. I understand that my authorization for the photograph(s) may not be revoked.

Billing Information
